ABSTRACT

Objective The purpose of this study was to analyze the effect of the coronavirus disease 2019 (COVID-19) on injury prevalence in National Collegiate Athletic Association (NCAA) varsity athletes following mandatory state-issued stay-at-home orders in March 2020. A secondary objective was to evaluate the relationship between COVID-19 infection and injury prevalence. Methods The respondents were recruited during pre-participation evaluations held at a Division I university in California in the summer of 2021, as well as via emails shared by athletic trainers at the institution. Data was collected using the Qualtrics Survey Platform (Qualtrics, Provo, UT). For all questions regarding the effects of the COVID-19 pandemic, the participants were asked to compare March 2020-March 2021 ("post-pandemic") to March 2019-March 2020 ("pre-pandemic"). Injury was defined as a physical complaint or condition sustained by an athlete during participation in training or competition that resulted in at least one missed day of practice or competition. The study participants were also asked to disclose their history of laboratory-confirmed COVID-19 infection. Results One hundred forty-six respondents completed the survey, with a 72.3% response rate. Of the respondents, 33.6% (n=49) reported sustaining at least one injury in the year preceding the pandemic, whereas 45.2% (n=66) of respondents reported sustaining at least one injury within the first year of the pandemic, a 34.5% relative increase in injuries (RR=1.35; 95% CI=1.01, 1.80). There was no significant difference in the number of upper body (RR=1.64; 95% CI=0.8, 3.34; p=0.177) versus lower body (RR=1.31; 95% CI=0.94, 1.82; p=0.11) injuries before and after the pandemic onset. Thirty-two respondents reported a history of COVID-19 infection. The athletes who reported a prior COVID-19 diagnosis were no more likely than the athletes with no prior COVID-19 diagnosis to obtain an injury from March 2020 to February 2021 (p=0.85). Conclusion This study indicates that the COVID-19 pandemic and stay-at-home orders were associated with a greater risk of injury in this cohort of collegiate athletes. Interestingly, a history of laboratory-confirmed COVID-19 infection was not associated with increased risk of injury.

ABSTRACT

Alcohol-associated hepatitis (AH) refers to liver injury from alcoholic intake that usually occurs after years of heavy alcohol abuse. Frequent, heavy alcohol consumption causes hepatic inflammation, fibrosis, and cirrhosis. Some patients develop severe AH, which carries high short-term mortality and is the second most common reason for adult liver transplants (LTs) worldwide. We present one of the first cases of a teenager diagnosed with severe AH that led to LT evaluation. Our patient was a 15-year-old male who presented with epistaxis and 1 month of jaundice after 3 years of heavy daily alcohol abuse. In collaboration with our adult transplant hepatologist colleagues, we initiated a management plan that consisted of treating acute alcohol withdrawal, steroid utilization, mental health support, and LT evaluation.

ABSTRACT

Objectives: After the third wave of coronavirus disease (COVID-19), by mid-February 2021, approximately 0.16% of the population was confirmed positive, which appeared to be one of the lowest rates worldwide at that time. However, asymptomatic transmission poses a challenge for COVID-19 surveillance. Therefore, a community-based serosurvey of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection was conducted to understand the effectiveness of Korea's strong containment strategy. Methods: We collected 5,002 residual sera samples from January 30 to March 3, 2021 from 265 medical facilities in Seoul, 346 in Kyunggi-do' and 57 in Incheon. Among them, 60 samples from tertiary institutions were excluded. We defined the sub-regions according to the addresses of the medical facilities where the specimens were collected. Elecsys Anti-SARS-CoV-2 was used for the screening test, and positivity was confirmed using the SARS-CoV-2 sVNT Kit. Prevalence was estimated using sampling weight and the Wilson score interval for a binomial proportion with a 95% confidence interval. Results: Among the 4,942 specimens, 32 and 25 tested positive for COVID-19 in the screening and confirmatory tests, respectively. The overall crude prevalence of SARS-CoV-2 antibody was 0.51%. The population-adjusted overall prevalence was 0.55% in women and 0.38% in men. The region-specific estimation was 0.67% and 0.30% in Gyeonggi-do and Seoul, respectively. No positive cases were detected in Incheon. Conclusion: The proportion of undetected cases in South Korea remains low. Therefore, an infection control strategy with exhaustive tracing and widespread pre-emptive testing appears to be effective in containing the spread of the virus in the community.

This study aimed to assess changes in mental health and substance use in National Collegiate Athlete Association (NCAA) Division I athletes in Southern California during the COVID-19 pandemic. Methodology An online survey was created using the Qualtrics software (Qualtrics, Provo, Utah). NCAA Division I athletes in Southern California completed preseason surveys querying indices of mental health, substance use, and injury in the year before the COVID-19 pandemic (March 2019 to March 2020) and during the pandemic (March 2020 to March 2021). The athletes filled out the survey from June 2021 to September 2021. Participants were asked how likely they were to agree with the following statements: I have felt physically prepared for athletic competitions, I have been satisfied with my mental health, and I have had adequate sleep. Participants were also asked to compare their substance use between the two time periods. Sociodemographic information regarding participants' age, gender, sports team, as well as year in sport and school was also collected. Group comparison analyses were performed using Fisher's exact test. Correlations between mental health measures and other variables were examined using Spearman's correlation coefficients. Results A total of 189 athletes completed the survey (out of the 259 surveys that were started). Females were significantly less likely to feel satisfied with mental health (p < 0.01) and physically prepared for sport (p < 0.01). Across all respondents, satisfaction with mental health was positively correlated with adequate sleep (p < 0.01) and physical preparedness for sport (p < 0.01) and negatively correlated with injury (p < 0.05). There was no significant correlation between mental health status and history of COVID-19 infection (p = 0.84). The vast majority of athletes reported no significant change in substance use pre- to post-pandemic, with no differences according to sex. Conclusions The COVID-19 pandemic had a differential impact on the mental health of female versus male NCAA athletes. Mental health was correlated with sleep, physical preparedness, and being injury-free but not with a history of COVID-19 infection. Despite reports indicating increased substance use in the general population, athletes in this group reported no change in licit and illicit substance use.

ABSTRACT

COVID-19 exposed the vulnerability of compact cities against shock events. As the impact of COVID-19 not only persists, but also expands throughout the world, this study questions whether the compact city model would be sustainable in the post-COVID-19 era. As such, this study examines the dynamics among major COVID-19 outbreak events, government interventions, and subway ridership in two compact cities, Seoul and New York City. Then, to gain thorough understanding of the impact of risks on compact urban form, it narrows the scope to Seoul in comparing subway ridership patterns in 2019 and 2020, and identifying characteristics that affect the volatility of subway ridership levels. The results affirm that individual mobility, COVID-19 outbreaks, and government interventions are closely related, and reveal that the extent of social distancing measures in compact cities is limited. This finding aligns with existing literature that link diseases transmission with dense population and mixed land use, accentuating the vulnerability of the compact city model against shocks. As a result, a multidimensional urban planning approach that incorporates polycentric and decentralized urban form is recommended to effectively and sustainably control disease outbreaks in compact cities.

The editors-in-chief of the Annals of Regional Science offer an overview and analysis of recent developments at the journal from January 2020 through December 2021, a time period hampered by the COVID-19 pandemic. Annal's Impact Factor increased substantially to 2.646 in 2020. Moreover, submissions increased from pre-COVID times. A new development is the shifting of source regions for articles accepted for publication. For the first time, China tied with the USA to lead the distribution of acceptances by country. Special Issues continue to be important components of the journal.

ABSTRACT

Background: The pandemic caused by severe acute respiratory coronavirus virus 2 (SARS-CoV-2) has dramatically increased cheshospitalizations, and it is often difficult to determine whether there is a bacterial or fungal coinfection at time of presentation. In this study, we sought to determine the rates of coinfection and utilization of antibiotics in SARS-CoV-2 disease. Methods: Retrospective chart review of patients hospitalized with COVID-19 pneumonia from April 13, 2020, to July 14, 2020. Results: In total, 277 patients were hospitalized for COVID-19 pneumonia during this period. Patients that received antibiotics within 48 hours of presentation were more likely to be febrile (59.3% vs 41.2%; P = .01) and to have leukocytosis (23.9% vs 5.9%; P < .01) and were less likely to have a procalcitonin level <0.25 ng/mL (58.8% vs 74.5%; P = .04). In total, 45 patients had positive blood cultures collected during hospitalization, 16 of which were clinically significant. Of the clinically significant blood cultures, 5 were collected <48 hours of admission. Moreover, 18 sputum cultures were clinically significant, 2 of which were collected within 48 hours of admission. Conclusion: Bacterial and fungal coinfections in COVID-19 appear to be rare on presentation; thus, this factor may be a good target for enhanced antibiotic stewardship.

ABSTRACT

With the spread of COVID-19, significant emphasis has been placed on mitigation techniques such as mask wearing to slow infectious disease transmission. Widespread use of face coverings has revealed challenges such as mask contamination and waste, presenting an opportunity to improve the current technologies. In response, we have developed the Auto-sanitizing Retractable Mask Optimized for Reusability (ARMOR). ARMOR is a novel, reusable face covering that can be quickly disinfected using an array of ultraviolet C lamps contained within a wearable case. A nanomembrane UVC sensor was used to quantify the intensity of germicidal radiation at 18 different locations on the face covering and determine the necessary exposure time to inactivate SARS-CoV-2 in addition to other viruses and bacteria. After experimentation, it was found that ARMOR successfully provided germicidal radiation to all areas of the mask and will inactivate SARS-CoV-2 in approximately 180 s, H1N1 Influenza in 130 s, and Mycobacterium tuberculosis in 113 s, proving that this design is effective at eliminating a variety of pathogens and can serve as an alternative to traditional waste-producing disposable face masks. The accessibility, ease of use, and speed of sanitization supports the wide application of ARMOR in both clinical and public settings.

ABSTRACT

Fine particulate matter (PM2.5) is one of the main air pollution problems that occur in major cities around the world. A country’s PM2.5 can be affected not only by country factors but also by the neighboring country’s air quality factors. Therefore, forecasting PM2.5 requires collecting data from outside the country as well as from within which is necessary for policies and plans. The data set of many variables with a relatively small number of observations can cause a dimensionality problem and limit the performance of the deep learning model. This study used daily data for five years in predicting PM2.5 concentrations in eight Korean cities through deep learning models. PM2.5 data of China were collected and used as input variables to solve the dimensionality problem using principal components analysis (PCA). The deep learning models used were a recurrent neural network (RNN), long short-term memory (LSTM), and bidirectional LSTM (BiLSTM). The performance of the models with and without PCA was compared using root-mean-square error (RMSE) and mean absolute error (MAE). As a result, the application of PCA in LSTM and BiLSTM, excluding the RNN, showed better performance: decreases of up to 16.6% and 33.3% in RMSE and MAE values. The results indicated that applying PCA in deep learning time series prediction can contribute to practical performance improvements, even with a small number of observations. It also provides a more accurate basis for the establishment of PM2.5 reduction policy in the country.

ABSTRACT

There is concern that the global burden of coronavirus disease of 2019 (COVID-19) due to severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) infection might yield an increased occurrence of Guillain-Barré syndrome (GBS). It is currently unknown whether concomitant SARS-CoV-2 infection and GBS are pathophysiologically related, what biomarkers are useful for diagnosis, and what is the optimal treatment given the medical comorbidities, complications, and simultaneous infection. We report a patient who developed severe GBS following SARS-CoV-2 infection at the peak of the initial COVID-19 surge (April 2020) in New York City and discuss diagnostic and management issues and complications that may warrant special consideration in similar patients.
